A concierge in a practice can do real harm if it bluffs — and most “AI chat” does, then gets quietly retired after it invents an answer in front of a client. So Fern is engineered — to OWASP's LLM Top-10 — so that the honest thing is the only thing she can do.

No live internet, no wandering. Fern can only reach the vetted knowledge vault your practice approves — and nothing else.

Refuse by default

If the answer isn't in her vault, she says so and routes to you. A clean “I'll check and confirm” beats a plausible guess.

The law, right

PHIPA and PIPEDA govern here — not HIPAA. HIPAA is US law and doesn't apply to an Ontario practice. A vendor who leads with “HIPAA-compliant” is telling you they haven't read the room. As an independent hygienist you're the health-information custodian; Fern works as your agent — data minimised, kept in Canada, and never fed into a public AI tool.

Your College, too — the CDHO, not the RCDSO. Fern is built to the CDHO's AI guidance: she discloses she's AI, offers a human alternative for anyone who'd rather not use it, and logs that consent. No — and not because we asked it nicely. Fern is scope-locked to answering, booking, and routing. She never diagnoses, never assesses, never prescribes. Anything clinical is handed to you, and true emergencies fire a fixed 911/ER script the model can't override.
If Fern gets something wrong, who's liable — and does this keep me onside with the CDHO?+
You stay the accountable clinician and custodian — the CDHO expects that, and nothing here changes it. What changes is your exposure: Fern never diagnoses, speaks only from facts you approved, refuses when she's unsure, and logs every exchange — so she's engineered to reduce the risk of a misstep, not add one. She's built to the CDHO's AI guidance, and everything she says stays inside advertising you've approved.
Can Fern message my lapsed clients to bring them back — is that CASL-legal?+
Yes, correctly. Recall and reactivation are commercial messages under CASL, so Fern only messages where consent is on file: she captures express, timestamped consent at intake, puts sender identification and a working unsubscribe on every message, and holds back clients outside the two-year implied-consent window until you have express consent for them. The reactivation that's legal to send, sent — the rest flagged, not fired.
Is this HIPAA-compliant?+
HIPAA is US law and doesn't apply to an Ontario practice — that's the giveaway question for a vendor who hasn't done their homework. Fern is built to PHIPA (Ontario) and PIPEDA: data minimised, kept on Canadian servers, never sold or pooled, never fed into a public AI tool, and auditable line by line. You're the custodian; Fern is your agent.
How does Fern handle a genuine emergency?+
Deterministically — the language model is never in that loop. Airway trouble, facial swelling, uncontrolled bleeding, trauma: hard-coded tripwires fire a fixed “call 911 or go to the ER now” script. Fern can't decide to handle an emergency in conversation, because the model never gets the chance.
What happens when she doesn't know something — and can I edit what she says?+
She says so. Fern is closed-book: if an answer isn't in the knowledge you approved, she doesn't improvise — she tells the client she'll have you confirm and logs it for follow-up. And that knowledge is yours to edit, line by line; nothing goes live until you've read and signed off. A clean hand-off beats a confident guess, every time.
Do I need a specific website, a PMS, or new software to run this?+
No. If you run Jane, Open Dental, or just a calendar, Fern works alongside it — she hands you a fully-formed booking and, where the tools allow, reads your free/busy; she never writes to your chart. Run nothing but a phone and paper? Fern handles booking and recall outright. And if you have no website — or only a Facebook page — the owned site is a module we build as one system with the concierge. Hand-offs land in the inbox you already watch: no new dashboard, nothing to learn.
Can Fern actually book — and what if I need something you haven't listed?+
Today Fern hands you a fully-formed booking request — name, reason, contact, preferred times, and for a home visit the resident and facility details a family member gives on their behalf — deliberately without writing to your chart, so there's less to secure. Want deeper booking, recall automation, or a specific integration? That's the advantage of owning the build rather than renting a fixed product: we scope it to your practice and add it when it earns its place. Full voice is on the roadmap, held until it can be as honest as the rest of Fern — never a recorded guess.
Does Fern speak French, or other languages?+
Yes — Fern answers in your client's language, with the same grounded facts and the same honest refusals in each one. She launches in English, French, and Spanish, each built on knowledge you review and approve — never machine-translated on the fly, because a mistranslated health answer is exactly the risk we design out. Need another language your community speaks? Because you own the build, we add it deliberately, held to the same review.
What do my clients see — and do they know it's AI?+
A concierge that matches your practice — not a generic chatbot dropped on top. Fern introduces herself in plain language as your practice's concierge and never impersonates a person or a clinician, so clients always know what she is — and anyone who'd rather not use AI is offered a human path instead. That's exactly what the CDHO's AI guidance asks: disclose the AI, offer an alternative, log the consent. Fern does, by default.
Do I own it, or am I renting?+
Your choice — and either way, the asset is yours. Own the build outright, with an optional month-to-month care line you can cancel anytime (the site stays yours). Or, if you'd rather spread it, a lower entry with a small monthly on a short term — the commitment is to the service, never a lock on your own site or data.
